Clinic Information
In your first visit, please be prepared to be in the office for at least 30 minutes early to gather information about your condition and your health history. Upon entering the clinic, you will be given a Patient Information Form to complete. Usually this will take less than 10 minutes. This information will be used to evaluate your general health as well as your chief complaint in order to make an accurate diagnosis and an effective treatment plan. When the Patient Information Form is complete, chiropractic doctor well perform a history taking with you by one-on-one basis. At this time the Doctor will ask you several questions regarding your general health, past history, and chief complaint. Also, the Doctor will answer any questions you may have. As soon as the history is completed, doctor will perform a comprehensive exams such as a physical, orthopedic, and chiropractic examinations.
After doctor has collected all the necessary information, he will prepare a report of findings and a treatment plan. If additional information or tests are necessary such as blood test or X-rays, you will be referred to the appropriate procedures.
